Short-term home from hospital care is a temporary support service designed to help individuals recover safely and comfortably at home after a hospital stay. We aim to arrange care as quickly as possible, often within 24 to 48 hours of hospital discharge. We work closely with hospital discharge teams, families, and healthcare professionals to ensure a smooth transition and timely support.
